How to Apply for Sibling or Twin Data Collection Projects

We’re here to help you through the application process for sibling and twin data collection projects. This guide will walk you through each step so you can easily apply and invite your siblings or twins to participate with you.

Before You Begin

You’ll need:

  • The QR code or URL shared by your recruiter
  • Your OneForma login credentials
  • A sibling or twin from the eligible states and cities (if applicable)

Step 1: Access the Application

  1. Open the QR code or URL that your recruiter shared with you
  2. Use any web browser to access the link
  3. Log in using your OneForma credentials

You’ll be automatically redirected to the job application page once you’re logged in.

Step 2: Complete the Project NDA

The first step in most projects is reviewing and signing the project NDA.

  1. Read through the project NDA carefully
  2. Scroll to the bottom of the document
  3. Provide your e-signature
  4. Click Submit All Signatures to proceed

Step 3: Answer Demographic Questions

Now you’ll answer some questions about yourself and your eligibility.

  1. Carefully answer all demographic questions as they appear
  2. When you see the question “Do you have a sibling or twin from the following states and cities to join this project with you?”, read the list of eligible locations
  3. Select “Yes” if you have a sibling or twin from one of the listed cities
  4. Complete all remaining questions in the survey
  5. Read through the privacy disclosure document
  6. Provide your consent by selecting the checkbox
  7. Click Next to continue

Important: Make sure you answer every question completely. If your responses don’t meet the project’s eligibility criteria, your application will be disqualified and you won’t be able to continue.

The next step is reviewing and signing the informed consent form.

  1. Read through the Inform Consent Form carefully
  2. Scroll to the bottom of the document
  3. Provide your e-signature
  4. Click Submit All Signatures to proceed

What Happens Next?

Once you’ve completed all the steps, your application will be reviewed. If you’re eligible and selected, you’ll receive instructions on how to invite your sibling or twin to participate.
